GOLDEN CHIROPRACTIC PRIVACY POLICY STATEMENT
GOLDEN CHIROPRACTIC (herein referred to as “GOLDEN CHIROPRACTIC”) is committed to keeping the personal information of its customers accurate, confidential, secure and private. Our Privacy Policy has been designed to inform clients, customers, and third parties of GOLDEN CHIROPRACTIC (herein referred to as “Customers”) of our commitment and recognition to our obligation to meet the spirit and terms of the Canadian federal Personal Information Protection and Electronic Documents Act (PIPEDA).

Our Privacy Policy describes the principles by which GOLDEN CHIROPRACTIC protects the privacy of personal information in its possession. It addresses the reasons why such information is collected, how it is used, how its confidentiality is protected and outlines the customers’ rights in respect of this information.
The Privacy Policy applies to personal information about the customers of GOLDEN CHIROPRACTIC and other individuals that is collected, used or disclosed by GOLDEN CHIROPRACTIC. This Privacy Policy does not apply to information about corporate customers and does not apply to information about the employees of GOLDEN CHIROPRACTIC. GOLDEN CHIROPRACTIC does, however, protect the confidentiality of such information in accordance with the law and our own policies.
Personal Information Protection Principles
1. Accountability
GOLDEN CHIROPRACTIC is accountable for the protection of all personal information within the Golden Chiropractic's possession or control, including any personal information that has been transferred to a third party for regulatory, legal or processing purposes. GOLDEN CHIROPRACTIC will require a comparable level of protection of this information from its third-party relations.

2. Identifying Purposes
Personal information that GOLDEN CHIROPRACTIC collects from customers includes:
· the customer’s name and address and other contact information, such as telephone numbers, email address.
Additional purposes for collecting personal information may be identified to an individual before or at the time of collection. However, at a minimum GOLDEN CHIROPRACTIC will collect personal information for the following purposes:
· To verify the customer’s identity and payment information;
· To determine the customer’s eligibility for products or services;
· To provide the customer with a service/product and to bill the customer for that service;
· To respond to the customer’s inquiries about fees paid or billing;
· To prevent fraud with respect to both the customer and Golden Chiropractic;
· To meet legal, regulatory and settlement requirements.
WHAT DATA DO WE COLLECT ABOUT YOU, FOR WHAT PURPOSE AND HOW DO WE PROCESS IT
Personal data means any information capable of identifying an individual. It does not include anonymized data.
We may process the following categories of personal data about you:
Communication Data that includes any communication that you send to us whether that be through the contact form on our website, through email, text, social media messaging, social media posting or any other communication that you send us. We process this data for the purposes of communicating with you, for record keeping and for the establishment, pursuance or defence of legal claims. Our lawful ground for this processing is our legitimate interests which in this case are to reply to communications sent to us, to keep records and to establish, pursue or defend legal claims.
Customer Data that includes data relating to any purchases of goods and/or services such as your name, title, billing address, delivery address email address, phone number, contact details, purchase details and your card details. We process this data to supply the goods and/or services you have purchased and to keep records of such transactions. Our lawful ground for this processing is the performance of a contract between you and us and/or taking steps at your request to enter into such a contract.
Sensitive Data
Our website generally does not collect any Sensitive Data* about you. Sensitive data refers to data that includes details about your race or ethnicity, religious or philosophical beliefs, sex life, sexual orientation, political opinions, trade union membership, information about your health and genetic and biometric data. We do not collect any information about criminal convictions and offences. *Please note that it is possible that someone could provide health information when filling out a form requesting a service with a practitioner. This is completely voluntary and we will protect the privacy of such information when it may be provided.
HOW WE COLLECT YOUR PERSONAL DATA
We may collect data about you by you providing the data directly to us (for example by filling in forms on our site or by sending us emails).
NOTE: We may receive anonymous data analytics from third parties such as Google or Squarespace.
3. Consent
The knowledge and consent of an individual are generally required for the collection, use or disclosure of personal information and GOLDEN CHIROPRACTIC will seek to obtain consent before or when it collects, uses or discloses personal information about an individual. An individual can provide consent to the collection, use and disclosure of personal information about them expressly or implicitly. However, GOLDEN CHIROPRACTIC will collect, use or disclose personal information without an individual's knowledge and consent only in limited circumstances and as permitted by law, such as in the case of an emergency where the life, health or security of an individual is threatened. Subject to certain legal and contractual restrictions and reasonable notice, an individual can refuse or withdraw their consent to the collection, use or disclosure of personal information about them at any time.
All customers are required to have read this Privacy Policy and agree to it prior to any service being offered, and are provided with information regarding GOLDEN CHIROPRACTIC’s collection, use and disclosure of their personal information when purchasing a product or service.
4. Limiting Collection
GOLDEN CHIROPRACTIC limits the amount and type of personal information it collects to that which is necessary for the services it provides and as permitted by law. Each affiliate of GOLDEN CHIROPRACTIC will be responsible for its own collection, use and disclosure of information. Personal Information will be collected using procedures that are fair, transparent and lawful.
5. Limiting Use, Disclosure and Retention
GOLDEN CHIROPRACTIC will only use the personal information for the purpose for which it was collected as identified in principle #2, unless consent is given by the individual to use or disclose it for another purpose or as is required by law. GOLDEN CHIROPRACTIC will develop explicit retention periods for closed accounts, after which the personal information will be destroyed or made anonymous.
Under certain exceptional circumstances, GOLDEN CHIROPRACTIC may have a legal duty or right to disclose personal information without the individual's knowledge or consent.
DATA SECURITY
We have put in place security measures to prevent your personal data from being accidentally lost, used, altered, disclosed, or accessed without authorization. We also allow access to your personal data only to those employees and partners who have a business need to know such data. They will only process your personal data on our instructions and they must keep it confidential.
We have procedures in place to deal with any suspected personal data breach and will notify you and any applicable regulator of a breach if we are legally required to.
6. Accuracy
GOLDEN CHIROPRACTIC shall take all reasonable steps to ensure that all personal information will be kept accurate, complete and up to date.
Individuals may challenge the accuracy and completeness of personal information about them and have it amended, as appropriate.
7. Safeguards
In executing its responsibilities with respect to the confidentiality of personal information, GOLDEN CHIROPRACTIC will employ a number of safeguards, appropriate to the sensitivity of the information, to protect personal information against loss or theft, as well as unauthorized access, disclosure, copying, use, or modification. Such safeguards will include physical measures, organizational measures and technological measures, for example locked filing cabinets, restricted access to office, limiting access on a “need to know” basis and the use of passwords and encryption. Procedures for implementing these measures will be communicated to all employees, contracted persons, and third parties to ensure compliance with this principle.
DATA RETENTION
We will only retain your personal data for as long as necessary to fulfil the purposes we collected it for, including for the purposes of satisfying any legal, accounting, or reporting requirements.
When deciding what the correct time is to keep the data for we look at its amount, nature and sensitivity, potential risk of harm from unauthorized use or disclosure, the processing purposes, if these can be achieved by other means and legal requirements.
For tax purposes the law requires us to keep basic information about our customers (including Contact, Identity, Financial and Transaction Data) for seven years after they stop being customers/clients.
In some circumstances we may anonymize your personal data for research or statistical purposes in which case we may use this information indefinitely without further notice to you.
YOUR LEGAL RIGHTS
Under data protection laws you have rights in relation to your personal data that include the right to request access, correction, erasure, restriction, transfer, to object to processing, to portability of data and (where the lawful ground of processing is consent) to withdraw consent.

We may need to request specific information from you to help us confirm your identity and ensure your right to access your personal data (or to exercise any of your other rights). This is a security measure to ensure that personal data is not disclosed to any person who has no right to receive it. We may also contact you to ask you for further information in relation to your request to speed up our response.
We try to respond to all legitimate requests within one month. Occasionally it may take us longer than a month if your request is particularly complex or you have made a number of requests. In this case, we will notify you.
If you are not happy with any aspect of how we collect and use your data, you have the right to complain to our Privacy Officer at EMAIL. We should be grateful if you would contact us first if you do have a complaint so that we can try to resolve it for you.

9. Individual Access
Any customer of GOLDEN CHIROPRACTIC can request access to the personal information about them that GOLDEN CHIROPRACTIC has in its possession or control. Any customer may request that their personal information be amended for purposes of accuracy and completeness.
